  1. In medicine we have many safeguards and checks to try to assure that errors do not happen, and if they do on one level hopefully they are caught on another level of security. When medical errors inevitably do happen it is only because there was a breakdown on all the different levels of security. We call it the Swiss cheese effect. If one layers multiple slices of Swiss cheese together, depending on how many holes in the Swiss cheese and how many layers there may be a way to get through all of it. I think the original question is an example of this. In the church as one rises through the ranks of leadership he would go through multiple “slices of Swiss cheese“ as he had various interviews. Depending on the interviewers they may not have been as in touch with the spirit at the time (more hole-y Swiss cheese [not more holy ?]). They may have received a premonition and asked more about it but if the interviewee is lying they may have felt uncomfortable to keep inquiring, or the spirit let it go altogether and let the lying individual reap his own rewards. We have many historical examples of interviewing leaders that were much less “hole-y” (very Holy), who felt the premonition to ask again and again about something, even postpone the rest of the interview until the individual came back and confessed what they were lying about, that takes guts to do, on both sides! Or possibly at the time the person was not having any gross misconduct and can answer truthfully, the spirit would not likely warn about their possibility for evil in the future and hence like Judas one could be righteous at one time, receive the call, and fall from righteousness. Fact is that we’ve had 100’s of thousands of church leaders and millions of missionaries throughout the church’s history, the Swiss cheese is not very hole-y and there are very many layers, inevitably something squeezes through the cracks and the world loves to put a spotlight on it but in reality it is a rare anomaly.
